Updated Army Military Funeral Honors Request Form

This message if from our contact with the Army Military Honors Program.

Attached to this email is a copy of the updated Army Military Funeral Honors Request Form. This form will request honors for Active Duty, Army Reserve, and Army National Guard Veterans.

Any time a funeral home, family, or friend would like to request military honors, they can fill out this PDF file and send it to the email address listed at the bottom of the second page (ng.in.inarng.list.j1sfp-fh@army.mil), with instructions.

Part of my job as a leader in this program is to help educate all people on honors, so if you may have any questions at all, please email or call any of the below contacts.

Lastly, please share this form with all funeral homes, family members you may know, in order for us to honor every Army veteran possible.
